Survey five stars question type

This module extends the Survey app with a new question type called "Five Stars Rating". When you build a survey, you can pick this type for a question, and respondents see five clickable stars instead of a text box or multiple-choice list.

• In the survey question form, a preview of the stars appears when the Five Stars Rating type is selected.
• On the survey page, clicking a star fills in the rating; the value is stored as a number between 0 and 5.
• If the question is mandatory, an answer of 0 or an empty answer is rejected with the configured error message.
• Answers outside the 0–5 range are rejected with "Answer is not in the right range".
• The system also checks saved answers: a five-star question must have a numeric answer, and mandatory questions require a value strictly above 0 and up to 5.
• In survey statistics, the module shows the most common ratings and the count of correct answers, using the same numerical summary as number questions.
• In printed surveys and per-respondent results, the chosen star value is displayed.

The module touches the survey question, survey user input and survey user input line models, and adds templates for the question, statistics, results and print views.
